Enhanced Care Management (ECM) is a no-cost care coordination program for Medi-Cal members with complex health needs. Whether you’re managing chronic conditions, facing housing challenges, or need help coordinating care, we’re here to walk with you every step of the way.

Green Tree Wellness provides dedicated Care Coordinators who become your healthcare advocate; helping you schedule appointments, connect to resources, overcome barriers, and improve your quality of life.

Are You Eligible for ECM?

Green Tree Wellness serves Medi-Cal members in San Diego and Riverside Counties who meet one or more of the following needs:

  • Housing Instability: Experiencing homelessness or unstable housing
  • Chronic Health Conditions: Managing complex or multiple medical conditions
  • Frequent Hospital Visits: High emergency room or hospital use
  • Mental Health Support: Living with serious mental health needs
  • Substance Use Support: Working through substance use challenges
  • High-Risk Pregnancy: Pregnancy with medical complications
  • Foster Care or Child Welfare: Current or former foster youth needing support
  • Transitioning from Incarceration: Re-entering the community from jail or prison
  • Developmental Disabilities: Need extra support for developmental or intellectual disabilities
  • Social Barriers: Facing challenges with food, transportation, or other basic needs

Common Questions About ECM

Is there a cost for ECM services?

No. ECM is completely free for eligible Medi-Cal members. There are no fees, co-pays, or charges of any kind.

How long can I stay enrolled in ECM?

As long as you need and continue to qualify. ECM is not time-limited—we support you for as long as you benefit from the program.

Will I have the same Care Coordinator?

Yes! You’ll be assigned a dedicated Care Coordinator who will work with you throughout your enrollment. They’ll get to know you, your goals, and your challenges.

What if I need help outside of business hours?

Current members receive text-ready direct lines to their Care Coordinators. We also provide crisis resource numbers for after-hours emergencies.

Do I need a referral from my doctor?

No. You can refer yourself, or you may be referred by your health plan, doctor, hospital, or community organization.
