Creating Meaningful Outcomes for Members

At Green Tree Wellness, we believe that everyone deserves compassionate, personalized support when navigating complex health challenges. Our mission drives everything we do—from the way we hire and train our Care Coordinators to how we show up for members every single day.

We’re not just coordinating care. We’re building relationships, removing barriers, and empowering members to take charge of their health and wellbeing.

Why We Do This Work

Healthcare can be overwhelming—especially when you’re managing chronic conditions, facing housing instability, dealing with behavioral health challenges, or trying to navigate multiple providers and systems at once.

Green Tree Wellness was founded on a simple belief: No one should have to face these challenges alone.

We saw too many people falling through the cracks—missing appointments because they couldn’t arrange transportation, struggling to understand their medications, or not knowing where to turn for basic needs like food or housing. We knew there had to be a better way.

That’s why we created our Enhanced Care Management program. We provide the dedicated, consistent support that transforms healthcare from a confusing maze into a manageable journey. Our Care Coordinators don’t just make referrals—they walk alongside members, advocate for their needs, and stay committed until goals are achieved.

Today, we’re proud to serve Medi-Cal members in San Diego and Riverside Counties, partnering with Kaiser Permanente, Molina Healthcare, and Inland Empire Health Plan to deliver high-quality, person-centered care coordination.

Understanding Enhanced Care Management

Enhanced Care Management (ECM) is a no-cost, high-touch care coordination program designed to support Medi-Cal members with complex medical, behavioral, and social needs.

Launched as part of California’s CalAIM initiative, ECM represents a shift in how healthcare is delivered to vulnerable populations. Instead of fragmented services and rotating staff, members receive comprehensive, continuous support from a dedicated Care Coordinator who knows them, advocates for them, and stays with them throughout their journey.

What Makes ECM Different? 

Traditional Care Management:

  • Rotating case workers
  • Focused on immediate issues
  • Time-limited support

Enhanced Care Management:

  • Long-term, continuous support
  • Whole-person approach (medical, behavioral, social)
  • Dedicated Care Coordinator

  • Support for as long as needed

Our Role

Green Tree Wellness provides ECM services to Medi-Cal members enrolled with Kaiser Permanente, Molina Healthcare, and Inland Empire Health Plan (IEHP) in San Diego and Riverside Counties.

Our team provides consistent, person-centered support and walks members through the often-overwhelming steps of managing health conditions, accessing services, and overcoming hardships. We coordinate with doctors, specialists, behavioral health providers, community organizations, and county agencies to ensure members get the comprehensive support they need.

Comprehensive Support, Personalized to You

Our Care Coordinators provide hands-on, individualized support based on each member’s unique needs and goals. Here’s what that looks like:

Care Coordination & Navigation

We handle the logistics so you can focus on your health:

  • Scheduling and rescheduling appointments
  • Securing follow-up appointments and transportation
  • Communicating with providers, clinics, and specialists on your behalf
  • Coordinating between medical, behavioral health, and social service providers
  • Making sure everyone involved in your care is on the same page
Personalized Care Planning

Your care plan is built around YOUR goals:

  • Comprehensive assessment of your needs, strengths, and challenges
  • Collaborative goal-setting based on what matters most to you
  • Clear action steps and timelines
  • Regular progress monitoring and plan adjustments
  • Celebration of milestones and successes along the way
Social & Community Support

We connect you to resources that address your whole life:

  • Food assistance and nutrition programs
  • Housing support and shelter services
  • Clothing, diapers, and basic needs
  • Transportation for medical appointments and essential errands
  • Durable Medical Equipment (wheelchairs, walkers, hospital beds)
  • Financial assistance programs
  • Home modifications for safety and accessibility
  • Asthma remediation services

  • Support communicating with healthcare providers

Advocacy & Empowerment

We’re your partner and advocate:

  • Crisis intervention and problem-solving support
  • Coaching on health self-management
  • Help communicating your needs and preferences to providers
  • Support during difficult medical decisions
  • Connecting you to peer support and community resources
  • Building your confidence and skills to navigate healthcare independently
Consistent Communication

Your Care Coordinator is accessible and reliable:

  • Regular check-ins (at minimum biweekly, often more frequent)
  • In-person visits when needed
  • Home visits when appropriate
  • Available by text, phone, and email
  • Responsive to urgent needs and concerns
  • The same Care Coordinator throughout your enrollment—no handoffs
