About Raiza: Raiza began her career in direct care coordination, supporting members with complex medical, behavioral health, and social needs. Her strong rapport-building skills, attention to detail, and commitment to equitable care naturally led her into program management. Today, she oversees ECM operations across both San Diego and Riverside counties, supports Care Coordinators, and ensures members receive compassionate, high-quality, person-centered care.

Raiza also has a clinical foundation—she completed nursing school and is a licensed Registered Nurse in Brazil. This background deepens her understanding of whole-person care and enhances her ability to guide teams working with medically complex members.

She works in the community helping clients to navigate the health system, community resources, support groups and other resources that affect the client’s health and well-being. Shera’s focus as a Registered Nurse Case Manager has been with first time mothers and their families.

She is passionate about supporting members with chronic conditions and housing instability. She believes in meeting Clients where they are, giving them the tools, and knowledge, empowering them to take charge of their health while providing compassionate, unwavering support. Shera utilizes a holistic approach, incorporating clients and their support system along with education and preventative measures to help the client achieve their ultimate goals.

When you enroll in Enhanced Care Management, you’ll be matched with a Care Coordinator based on your location, language preference, and specific needs. Here’s what you can expect:

Consistency: Your Care Coordinator stays with you throughout your enrollment—no rotating staff or handoffs to new people.

Personalization: Your Care Coordinator takes time to understand YOUR unique situation, goals, and preferences.

Accessibility: You’ll have your Care Coordinator’s direct line and can reach them by phone, text, or email.

Responsiveness: Your Care Coordinator responds to your calls and messages promptly—same day for urgent needs.

Respect: Your Care Coordinator treats you with dignity, respect, and cultural sensitivity.

  • Minimum biweekly check-ins (more frequent when needed)

  • Bachelor’s degree in social work, nursing, public health, or related field (minimum)
  • Experience in case management, care coordination, or community health
