Are You Eligible for ECM?

Green Tree Wellness provides Enhanced Care Management (ECM) services to Medi-Cal members in San Diego and Riverside Counties who meet specific eligibility criteria.

To qualify for ECM, you must:

  • Be enrolled in Medi-Cal (California’s Medicaid program)
  • Be a member of one of our partner health plans:
    • Kaiser Permanente
    • Molina Healthcare
    • Inland Empire Health Plan (IEHP)
  • Meet one or more of the Populations of Focus (detailed below)

Important Note

You only need to meet ONE of the populations of focus criteria to qualify. If you’re facing multiple challenges, that’s okay—we’re here to support you with all of them.

CalAIM Populations of Focus

Enhanced Care Management is designed to support Medi-Cal members who face significant barriers to health and wellbeing. If you identify with one or more of the following situations, you may qualify for our program

HOUSING & STABILITY

Individuals Experiencing Homelessness or Housing Instability

You may qualify if you are:

  • Currently experiencing homelessness (unsheltered or in emergency shelter)
  • Living in transitional housing
  • Temporarily staying with friends or family due to housing loss
  • At risk of losing your housing
  • Living in substandard or unsafe housing conditions
  • Recently housed but at high risk of returning to homelessness

How we help: Our Care Coordinators connect you to housing resources, emergency shelter, rental assistance programs, and supportive housing services. We coordinate with housing providers and help navigate the often complex process of securing stable housing while managing your health needs.

BEHAVIORAL HEALTH NEEDS

Adults or Youth with Serious Mental Health Needs

You may qualify if you:

  • Have been diagnosed with serious mental illness (SMI) such as schizophrenia, bipolar disorder, major depression
  • Experience significant functional impairment due to mental health conditions
  • Have difficulty managing daily activities due to psychiatric symptoms
  • Have been hospitalized for mental health crises
  • Need coordination between mental health and medical providers

How we help: We coordinate your behavioral health services with your medical care, help you attend therapy and psychiatry appointments, connect you to peer support and community resources, and ensure your mental health treatment plan is integrated with your overall care.

BEHAVIORAL HEALTH NEEDS

Individuals with Substance Use Disorders

You may qualify if you:

  • Are currently struggling with substance use or addiction
  • Are in recovery and need ongoing support
  • Have recently completed a substance use treatment program
  • Need help accessing detox, residential treatment, or outpatient services
  • Are facing barriers to recovery due to housing, transportation, or other needs

How we help: We support your recovery journey by coordinating substance use treatment services, connecting you to sobering centers and recuperative care, helping with medication-assisted treatment (MAT) coordination, and addressing social needs that impact your recovery.

MEDICAL COMPLEXITY

Members with High Emergency Room or Inpatient Utilization

You may qualify if you:

  • Have frequent emergency room visits (multiple times per year)
  • Have had multiple hospital admissions
  • Experience difficulty managing care after hospital discharge
  • Return to the hospital shortly after being released
  • Use the ER for conditions that could be managed in primary care

How we help: We provide intensive discharge planning, ensure you have follow-up appointments scheduled, coordinate transportation, help you understand post-hospital care instructions, and work to address the underlying causes of frequent ER use.

MEDICAL COMPLEXITY

Adults and Children with Chronic or Complex Medical Conditions

You may qualify if you have:

  • Multiple chronic conditions (diabetes, heart disease, COPD, asthma, etc.)
  • Complex medical needs requiring multiple specialists
  • Conditions requiring frequent monitoring and management
  • Difficulty managing medications or treatment plans
  • Challenges coordinating care between different providers
  • Pediatric chronic conditions requiring family support

How we help: We coordinate all aspects of your medical care, help schedule and attend specialist appointments, assist with medication management, communicate between your various providers, and help you understand and follow your treatment plan.

MEDICAL COMPLEXITY

Pregnant Individuals with High-Risk Pregnancies

You may qualify if you:

  • Have medical complications during pregnancy
  • Have chronic conditions that complicate pregnancy (diabetes, hypertension, etc.)
  • Are experiencing housing instability while pregnant
  • Have substance use concerns during pregnancy
  • Have limited prenatal care access
  • Face multiple barriers to a healthy pregnancy and delivery

How we help: We ensure you attend all prenatal appointments, coordinate with your OB and other specialists, connect you to prenatal nutrition programs (WIC), help with transportation to appointments, and provide support through delivery and postpartum care.

VULNERABLE POPULATIONS

Youth Involved with Child Welfare

You may qualify if you are:

  • Current foster youth
  • Former foster youth (up to age 26)
  • Youth aging out of foster care
  • Involved with child protective services
  • In kinship care or guardianship arrangements

How we help: We provide specialized support for youth navigating the child welfare system, coordinate with social workers and foster care providers, help with transition planning as you age out, connect you to education and employment resources, and ensure continuity of care during placement changes.

VULNERABLE POPULATIONS

Individuals Transitioning from Incarceration or Institutions

You may qualify if you are:

  • Recently released from jail or prison
  • Transitioning from a state hospital or psychiatric facility
  • Re-entering the community from juvenile detention
  • Coming from any institutional setting
  • On parole or probation and need health support

How we help: We support your successful re-entry by scheduling medical and behavioral health appointments, helping you access medications, connecting you to housing and employment resources, coordinating with parole/probation officers, and ensuring continuity of care during transition.

VULNERABLE POPULATIONS

Members with Developmental or Intellectual Disabilities

You may qualify if you have:

  • Developmental disabilities requiring additional support
  • Intellectual disabilities affecting daily functioning
  • Autism spectrum disorder
  • Conditions requiring Regional Center services
  • Complex medical needs related to developmental disabilities
  • Need for care coordination across multiple systems

How we help: We coordinate with Regional Centers, special education services, behavioral specialists, and medical providers. We help you and your family navigate complex systems, ensure you receive appropriate accommodations, and advocate for your needs.

SOCIAL DETERMINANTS OF HEALTH

Individuals Facing Significant Social Determinants of Health Barriers

You may qualify if you experience:

  • Food insecurity or lack of access to healthy food
  • Transportation barriers preventing medical appointment attendance
  • Difficulty affording medications or medical equipment
  • Language barriers affecting healthcare access
  • Lack of social support or isolation
  • Multiple competing priorities (childcare, work, caregiving)
  • Limited health literacy or understanding of the healthcare system
  • Immigration-related barriers to accessing services

How we help: We address the root causes affecting your health by connecting you to food assistance, arranging medical transportation, helping access financial assistance programs, providing interpreter services, connecting you to community resources, and removing barriers that prevent you from engaging in your healthcare.

SOCIAL DETERMINANTS OF HEALTH

Anyone Identified by Their Health Plan as Needing Intensive Care Coordination

You may qualify if:

  • Your health plan has identified you as needing enhanced support
  • Your doctor has referred you for care management
  • You have a care management team recommendation
  • You meet health plan criteria for high-risk members

Common Questions About Eligibility

I have multiple conditions—do I still qualify?

Yes! Many of our members face multiple challenges simultaneously. In fact, ECM is specifically designed for individuals with complex, overlapping needs. Whether you’re managing chronic illness while experiencing housing instability, or dealing with mental health needs and substance use recovery, we’re here to support you holistically.

I'm not sure if my situation is 'serious enough' for ECM. Should I still apply?

Absolutely. If you’re struggling to manage your health, facing barriers to care, or feeling overwhelmed by your situation, we encourage you to apply. Our team will assess your needs and help determine if ECM is the right fit. There’s no harm in applying, and we’d rather connect you to support early than have you struggle alone.

Can children receive ECM services?

Yes! Children with chronic or complex medical conditions, developmental disabilities, or involvement with child welfare can receive ECM services. Parents or guardians work with our Care Coordinators to ensure children receive comprehensive, coordinated care.

What if I don't speak English?

Language is never a barrier to receiving ECM services. We provide interpreter services in all languages at no cost to you. Many of our Care Coordinators are bilingual, and we ensure communication happens in your preferred language.

I'm undocumented—can I still get ECM?

If you’re enrolled in Medi-Cal, you may be eligible for ECM regardless of immigration status. Medi-Cal eligibility varies based on age and circumstances. Contact us to discuss your specific situation in a confidential, supportive environment.

Do I need a referral from my doctor?

No. You can refer yourself to our ECM program by completing our intake form or calling us directly. Referrals can also come from your health plan, hospital, social worker, or community organization—but you don’t need anyone’s permission to apply.

What if I'm already working with a case manager?

That’s great! We coordinate with your existing supports. ECM provides an additional layer of intensive care coordination and can work alongside case managers, social workers, and other support staff. We see ourselves as part of your team, not a replacement.

I'm about to be released from jail—can I enroll in ECM before I get out?

Yes! We can begin the enrollment process before your release to ensure you have support waiting for you. Successful re-entry planning includes healthcare coordination, and we’re here to help make that transition as smooth as possible.
