Yes, all in-person visits are by appointment only. Please call (858) 304-4030 to schedule. Many members prefer phone or home visits for convenience – we’re flexible and work around your schedule.
We respond to all inquiries within one business day. If you call during business hours, you’ll speak with someone immediately. If you submit a form or email outside of business hours, we’ll respond the next business day.
When you enroll, we match you with a Care Coordinator based on your location, language preference, and needs. If you have concerns about your assigned Care Coordinator, we’ll work with you to find the best fit. You can view our team members on our Our Team page.
We provide professional interpreter services in all languages at no cost to you. Many of our Care Coordinators are bilingual (English/Spanish), and we can arrange interpretation for any language. Let us know your language preference when you contact us.
Yes! Family members, social workers, healthcare providers, and community partners can contact us to make referrals or ask questions on behalf of potential members. We’ll need the member’s consent before enrolling them in services, but we’re happy to provide information to anyone who asks.

For privacy and security, please do not send sensitive health information via email. If you need to share private medical or personal information, we’ll provide secure methods during the intake process. General questions via email are always welcome.
We take all concerns seriously. If you have a complaint about our services, please contact our main line at (858) 304-4030 and ask to speak with a supervisor. You can also email concerns through the contact form. We’ll respond within 48 hours and work to resolve any issues. You also have the right to file a grievance with your health plan.
